Are you dissatisfied with something, or do you have a complaint about the service I provided? I would be happy to discuss this with you and look for a solution.
If you do not want or are unable to discuss the complaint with me, or if you would like independent support, you can contact the Healthcare Complaints Portal. The Complaints Portal for Healthcare is an external organization that I am affiliated with to help resolve dissatisfaction and complaints about my care.
If you have sent your complaint or dissatisfaction, the Healthcare Complaints Portal will assess whether it meets the conditions for handling it and which procedure would be appropriate. If certain information is missing or unclear, they will ask you to complete it and ensure that the complaints procedure is started properly.
Would you like to receive advice on how to deal with a situation you are dissatisfied with? In that case you can also contact the Healthcare Complaints Portal. This will not start a procedure yet.
Submit your complaint using the complaints form on the Healthcare Complaints Portal website. You can also send an e-mail to info@klachtenPortaalzorg.nl. Are you unsure which complaints procedure applies to you? The Healthcare Complaints Portal can also help you with that. The goal remains to resolve dissatisfaction and complaints.
